What does the day look like?
A Clinical Trials Pharmacist works within an Investigational Drug Service (IDS) unit at an academic hospital or dedicated phase 1 clinical facility, working a 37.5 to 40-hour week. About 35% of the time is spent on aseptic compounding and dispensing inside cleanrooms (Class A/B environment) using laminar flow hoods or isolators to prepare sterile IV infusions, monoclonal antibodies, cytotoxic agents, and double-blind masked doses. Another 25% goes to regulatory protocol reviews, examining Investigator's Brochures (IB) and IMPDs, and drafting pharmacy dispensing workflows. Cold-chain management and accountability take up 20%, verifying -80°C freezers and 2°C to 8°C cold rooms with Testo or Sensitech data loggers and logging stock in Vestigo, Epic Willow Research, or Cerner Millennium.
Monitoring visits with CRAs, auditing individual vials, and reporting SUSAR safety events account for 15% of the week. Direct patient counselling covers just 5%. Core shifts run Monday to Friday from 08:30 to 17:00, with on-call rotas (such as 1 weekend in 6) for temperature alarms, emergency unblinding, or urgent infusions. Night shifts occur rarely outside 24-hour phase 1 units. Rather than inventing molecules, over 80% of the day consists of regulatory logging, chain-of-custody tracking, sterile cleanroom compounding, and audit compliance.
