What does the day look like?
On bypass days you run extracorporeal circulation during heart surgery under a physician order: selecting and priming the circuit, oxygenator, pumps, cannulae, cardioplegia set and monitoring lines, running cardiopulmonary bypass, managing blood gases, anticoagulation, temperature, flows and pressures, giving blood products per protocol, documenting perfusion parameters, and supporting weaning and decannulation. Related duties include ECMO support, ventricular assist device support, intraoperative blood salvage and autotransfusion, paediatric and adult set-ups, and emergency action on device failure. Named systems include Stockert S5 and Stockert S3, LivaNova Essenz and C5, Medtronic Bio-Console and Affinity oxygenators, Getinge HL 40 and Cardiohelp ECMO, Terumo CAPIOX circuits, Sorin Stockert equipment, and hospital record systems such as MEDITECH.
As an aide or trainee the day is weighted toward preparation, cleaning, stocking, equipment checks, circuit assembly assistance, machine transport, quality assurance logs, and observation from the pump room. Independent bypass runs are a minority of aide time under direct supervision, with the majority taken by set-up, decontamination, inventory, documentation and standby. Scheduled cardiac lists come with nights, weekends and on-call cover, because aortic dissection, transplant and ECPR cases do not follow elective schedules. O*NET work context data for the matched occupation reports constant contact with others, very close physical proximity, daily time pressure and daily decision making, with consequence of error rated extremely serious.
