What does the day look like?
You run the heart-lung machine during cardiac surgery. While the surgeon works on the heart, you keep the patient's circulation and gas exchange going outside the body on cardiopulmonary bypass. The tasks are priming and checking the bypass circuit, managing physiological parameters on bypass, blood conservation and blood processing, reading blood gas and blood chemistry values, delivering cardioplegia, running mechanical support such as intra-aortic counter-pulsation, plus documentation, statistics, and research. Extended work includes ECMO, isolated limb perfusion for chemotherapy, and support for liver and organ transplantation.
The kit is the bypass console with roller or centrifugal pumps, the membrane oxygenator, heater-cooler units, cardioplegia delivery systems, activated clotting time monitors, and blood gas analysers. Most of the day is preparation, checks, monitoring, standby, and paperwork, with the bypass run as a concentrated block inside a longer operation. You work in a small theatre team with the surgeon, anaesthetist, and scrub nurse, carrying direct personal responsibility for the circuit. Hours are theatre hours plus on-call cover, with night and weekend emergency calls for acute cardiac cases, ECMO, and transplant support.
