What does the day look like?
You run extracorporeal circulation during open-heart surgery: you prime and drive the heart-lung machine — LivaNova S5, LivaNova S7, Medtronic Bio-Console 560, Terumo Advanced Perfusion System 1 or Getinge HL 40 — manage bypass, cardioplegia, anticoagulation by activated clotting time, blood gases and electrolytes on Radiometer ABL90 or Siemens RAPIDPoint 500, and wean from bypass in direct talk with the surgeon and anaesthetist. Records go into Epic or MEDITECH, pump data into LivaNova Connect or Medtronic VitalTrak.
Pump time with the heart stopped is 30% to 40% of an operating day; setup, checks, documentation, blood collection with Haemonetics Cell Saver Elite, ECMO rounds, ventricular assist device and intra-aortic balloon pump support, maintenance logs and stock take 60% to 70%. Work is hospital shifts with nights, weekends and on-call cover, with 1 in 4 or 1 in 5 rotas normal in small heart teams; emergency bypass and ECMO do not keep office hours.
