What does the day look like?
You run extracorporeal circulation during open-heart surgery: you assemble, prime and run the heart-lung machine, manage cardiopulmonary bypass flow, oxygenation and temperature, monitor anticoagulation with activated clotting time, manage blood gas and electrolytes, deliver cardioplegia, run cell salvage, and wean from bypass with the surgeon and anaesthetist. Named systems are the LivaNova S5 and Stockert S5 heart-lung machines, Terumo Advanced Perfusion System 1, Medtronic Affinity and LivaNova Inspire oxygenators, Sorin Stockert roller and centrifugal pumps, with hospital documentation in Epic or Cerner perfusion records.
The imagined part, hands on bypass in theatre, is roughly half the rostered day when cardiac lists run, and a smaller share in weeks dominated by checks, on-call cover, audit and training. The rest is preparation, circuit checks, stock and blood-product coordination, intra-aortic balloon pump and ECMO support, ventricular-assist-device standby, and standby cover for transplants and emergencies. Simulator sessions are training, not clinical service. The rota means early starts, late finishes, night and weekend on-call, and emergency call-in for dissections, transplants and ECMO retrievals.
