What does the day look like?
Your day happens next to the heart-lung machine. You run cardiopulmonary bypass for adult and paediatric cardiac surgery, plus extracorporeal membrane oxygenation, ventricular assist device support, intraoperative blood management, myocardial protection and perfusion charting. The named systems are the LivaNova S5 and LivaNova Essenz heart-lung machines, Medtronic Bio-Console and Bio-Pump, Getinge Cardiohelp for extracorporeal membrane oxygenation transport, Sorin Stockert 3T, Terumo CDI blood parameter monitors, ROTEM thromboelastometry, and electronic perfusion records inside Epic or Cerner.
At director level you add staffing and call rosters across sites, competency sign-off, perfusion protocol and policy ownership, equipment capital planning, contract review with device vendors, quality dashboards covering prime volumes, transfusion rates, acute kidney injury signals and neonatal outcomes, incident review, accreditation readiness, and liaison with cardiac surgeons, anaesthetists, intensive care and blood bank. In a system role the clinical share is a minority of hours, with management, meetings, recruitment, budgeting and cross-site standardisation taking the majority. In a 1-centre chief role the split reverses during operating hours. Hours follow cardiac surgery schedules with early starts, late finishes, weekend cover and night and weekend on-call for dissection, transplant, trauma and extracorporeal membrane oxygenation retrieval. On-call is continuous in small teams and rota-based in large teams. Night work and callback are structural parts of the job, not exceptions.
