What does the day look like?
You run the heart-lung machine during open-heart procedures: you prime and check the circuit, manage pump flow, oxygenation, temperature and anticoagulation, deliver cardioplegia, control blood gas and electrolytes, then wean from bypass together with the surgeon and anaesthetist. The work uses LivaNova and Sorin consoles, Terumo and Medtronic oxygenators and reservoirs, hemoconcentrators, cell salvage, activated clotting time monitors and inline blood gas monitoring, with the record kept in the hospital electronic record.
The bypass run itself is a minority of clock time. Preparation, checks, documentation, meetings, teaching and waiting between cases take a large share. Beyond bypass there is extracorporeal membrane oxygenation and ventricular assist device support, emergency standby, equipment maintenance and safety checks, blood conservation, audit and stock control of disposables. At senior grade you add trainee supervision, rota organisation, protocol authorship, incident review and liaison with surgery, intensive care and transfusion teams. Transplant, dissection, trauma and extracorporeal rescue do not keep office hours, so nights and weekends on call are structural.
