What does the day look like?
Standard contracts mandate 37.5 hours per week, typically Monday to Friday from 08:30 to 17:00. Night shifts are rare, and on-call duties occur mainly in acute stroke pathways, rapid-response emergency discharge teams, or executive on-call manager rotas. Many teenagers imagine spending 90% of their time playing recovery games, crafting, and doing physical exercises with cooperative patients. In reality, direct clinical work takes only 30% to 40% of the week, focusing on high-risk tertiary cases such as severe traumatic brain injury, complex polytrauma, advanced neurodegenerative conditions, or treatment-resistant psychiatric conditions.
The remaining 60% to 70% of working time involves strategic leadership, multidisciplinary meetings, clinical governance, mentoring staff, and research. Strategic leadership and service consultancy take 30% of your week, education and supervising Band 6 and Band 7 therapists take 15% to 20%, and clinical trials or audits take 15%. Daily clinical tools include standardized assessments such as Assessment of Motor and Process Skills (AMPS), Model of Human Occupation Screening Tool (MOHOST), Canadian Occupational Performance Measure (COPM), Functional Independence Measure (FIM), Barthel Index of Activities of Daily Living (ADL), and Loewenstein Occupational Therapy Cognitive Assessment (LOTCA). Work also requires electronic health record systems like SystmOne (TPP), EMIS Web, RiO, Epic Systems, Cerner Millennium, and statistical software like SPSS and R.
