What does the day look like?
You work 37.5 hours per week under a standard NHS full-time contract, typically scheduled from 8:00 to 17:00, Monday to Friday. Acute hospital trusts operate 7-day services where staff rotate through 1 in 4 weekends. Shifts in emergency departments and acute frailty assessment units can extend to 20:00. Night shifts and overnight on-call rotas do not form part of standard occupational therapy practice.
Direct patient contact and functional assessment take 40% to 50% of the working day. You observe patients performing Activities of Daily Living (ADLs) such as washing, dressing, meal preparation, and chair transfers. You administer cognitive tests like the MoCA and MMSE, measure joint range of motion with goniometers, evaluate grip strength with Jamar hydraulic dynamometers, and prescribe assistive equipment like hoists and wheelchairs. You also visit patients' homes to measure door clear openings and stair angles for fall hazards. The remaining time is administrative: 30% to 35% of the day goes to clinical documentation in Electronic Patient Record systems like Epic Systems or Cerner Millennium and drafting grant justification reports, while 15% to 20% is spent in multidisciplinary board rounds. Over 85% of the day involves physical handling, personal hygiene assessments, and legal paperwork.
