What does the day look like?
Between 30% and 45% of your shift is spent on clinical ward rounds with physicians and nurses across intensive care units (ICU), surgical, or acute medical wards. This involves screening patient charts for drug contraindications, calculating organ clearance using Cockcroft-Gault, and monitoring antimicrobial stewardship. Another 20% to 30% of the day goes into inpatient prescription screening and therapeutic drug monitoring for narrow-therapeutic-index drugs like vancomycin, aminoglycosides, or phenytoin using EHR software such as Epic, Cerner Millennium, or Dedalus.
The remaining time splits between supervising aseptic cleanrooms (ISO Class 5 laminar airflow hoods) preparing total parenteral nutrition (TPN) and cytotoxic chemotherapy (10% to 15%), discharge medicines reconciliation and patient counselling (10% to 15%), and documentation or Yellow Card reporting (10% to 15%). Shifts include late dispensary cover until 20:00 or 22:00, weekends, and overnight on-call rotations to authorize urgent antidotes or surgical agents.
