What does the day look like?
The working day involves clinical assessment, differential diagnosis, therapy planning, and direct sessions with children and adults. Paediatric caseloads cover speech sound disorders, stammering, developmental language disorder, and swallowing difficulties. Adult caseloads focus on recovery from stroke, head injury, Parkinson disease, dementia, and dysphagia. Clinicians use standardised language batteries, phonetic transcription, voice analysis software, alternative communication systems, and instrumental swallowing assessments such as videofluoroscopy coordinated with medical teams.
While direct therapy is the most visible element, a large share of paid hours goes to administrative tasks: writing clinical notes in NHS electronic record systems or Epic and Cerner portals, updating the Royal College online outcome tool, preparing Education Health and Care plan contributions, and managing waiting lists. The standard NHS working week is 37.5 hours on daytime rotas across clinics, schools, and hospital wards. Night shifts are not standard, and on-call duties are limited to acute dysphagia cover rotas.
