What does the day look like?
On an acute stroke and neurology ward, the day revolves around bedside swallow screening within hours of admission, full dysphagia assessments, and setting diet and fluid textures with nurses and catering. Clinicians assess aphasia, apraxia of speech, and dysarthria, screen cognitive communication after traumatic brain injury, evaluate capacity for consent, counsel families, and plan discharge to community rehabilitation. Clinical tools include videofluoroscopy swallow studies, fibreoptic endoscopic evaluation of swallowing, standardised batteries like the Boston Diagnostic Aphasia Examination, Western Aphasia Battery, and Sydney Language Battery, alongside hospital electronic records such as the NHS Electronic Patient Record, System C, EMIS, or Irish HSE iPMS, and augmentative communication devices from Tobii Dynavox.
The direct talking exercises most people imagine take up about 1/3 of the working day. The remaining 2/3 is swallow safety planning, documentation, multidisciplinary team meetings, referrals, and training nursing staff. Stroke units run 7 days a week, making scheduled weekend rota cover standard, along with acute on-call duties in some trusts. Regular night shifts are not typical.
