What does the day look like?
Direct clinical contact with patients takes up only 40% to 50% of your shift. You work with individuals recovering from acute stroke, complex physical trauma, neurological degeneration, or mental health crises to restore their daily functioning. Using tools like the Jamar hydraulic hand dynamometer, goniometers, and Semmes-Weinstein monofilaments, you run standardized assessments including the Barthel Index, COPM, FIM, AMPS, MoCA, and RBMT. You prescribe manual or powered wheelchairs and hoists, measure home doorways and stairs for discharge, and fabricate custom low-temperature thermoplastic splints.
The remaining 50% to 60% of your shift is administrative and supervisory. Around 30% to 35% of your day is spent writing legal clinical documentation and ordering equipment across EPR platforms such as Epic Systems, Cerner Millennium, SystmOne (TPP), EMIS Health, or Rio. Another 15% to 20% goes toward supervising Band 5 therapists and university students, audits, and multidisciplinary meetings. Standard weekly hours are 37.5 hours in the UK NHS, 37 hours in the Irish HSE, 38 hours in Australia, and 40 hours in the United States. Core shifts run Monday to Friday from 08:00 to 16:00 or 08:30 to 16:30. Rotas require 1 weekend shift every 4 to 6 weeks in acute units, with occasional twilight shifts ending at 20:00 or 22:00, while night shifts are rare.
