What does the day look like?
Standard full-time employment is 37.5 hours per week. In outpatient clinics and Community Mental Health Teams, work runs Monday to Friday from 9:00 to 17:00. In acute inpatient psychiatric wards and Crisis Resolution and Home Treatment Teams, services operate on 7-day rotas with 8-hour shifts (7:30 to 15:30 or 12:30 to 20:30) or 12-hour shifts (7:30 to 20:00). Night shifts are staffed almost exclusively by psychiatric nurses, but occupational therapists cover daytime weekend rosters and periodic crisis on-call rotations.
The daily routine differs sharply from popular assumptions. Peaceful 1-to-1 therapy and creative workshops occupy only 15% to 20% of the working day. In contrast, 40% to 50% of your time is consumed by clinical documentation, entering patient notes into electronic record systems like RiO (Servelec) or SystmOne, reporting incidents on Datix or Radar Healthcare, and drafting Mental Health Act tribunal reports. Another 25% to 30% involves managing acute psychiatric distress, active delusions, and severe behavioural crises. The remaining 10% to 15% goes to multidisciplinary team coordination and driving between community home visits. Clinical tasks include administering the Model of Human Occupation Screening Tool, Canadian Occupational Performance Measure, and Allen Cognitive Level Screen, as well as utilizing sensory rooms and weighted modalities to de-escalate acute agitation.
