What does the day look like?
You are not the person giving treatment, you are the bridge between the ward and what happens after it. In the morning you join discharge planning huddles with ward consultants, physiotherapists and charge nurses, then you run psychosocial and mental capacity assessments for frail elderly patients, stroke survivors, psychiatric admissions and trauma patients. The rest is statutory adult social care packages, palliative care placement, home adaptation equipment, nursing home step-down transfers, safeguarding reports on domestic abuse, institutional neglect and financial exploitation, plus case notes in Epic Systems, Cerner Millennium, System C CareFlow, Liquidlogic Adults' Social Care System (LAS), Mosaic (Access Group) and SystmOne, using Montreal Cognitive Assessment (MoCA), Barthel Index of Activities of Daily Living, Waterlow Scale and Care Act Assessment protocols.
About 15% of the day is bedside interviews and family meetings, 55% is inter-agency calls, case conferences and legal and funding paperwork, and 30% is digital casework and reporting. The standard week is 37.5 hours in the UK and 40 hours in the US and Germany. Core shifts are 08:30 to 16:30 or 09:00 to 17:00, Monday to Friday. Acute hospitals run 7-day cover, with 1 weekend in 4 on rota and on-call cover for the emergency department. Overnight ward shifts are rare, with emergency duty team on-call for psychiatric and safeguarding emergencies.
