What does the day look like?
An occupational therapist manages an autonomous clinical caseload across hospital inpatient wards, outpatient clinics, mental health units, or community home-care teams. Daily tasks include administering standardised cognitive and functional evaluations (such as MoCA, AMPS, Barthel Index, COPM, GAS, and FIM), prescribing and fitting assistive equipment (grab rails, shower chairs, hoists, specialized seating, and custom thermoplastic splints), and conducting home visits to measure doorways, stairs, and bathrooms for architectural modifications. You also attend daily Multi-Disciplinary Team (MDT) case conferences with physicians, nurses, and social workers to coordinate safe patient discharges.
While teenagers often imagine this profession consists of 80% playful therapy or crafts, direct patient contact actually accounts for only 35% to 45% of the working day. The remaining 55% to 65% is consumed by administrative duties: logging clinical notes in Electronic Patient Record systems (such as SystmOne, EMIS Health, Rio, Epic Systems, and Cerner), writing legal clinical reports, ordering equipment, and traveling between community home visits. In the UK NHS, standard full-time contracts specify 37.5 hours per week, typically Monday to Friday from 08:30 to 16:30 or 08:00 to 16:00. Acute hospital wards operate 7-day rotas requiring 1 weekend shift every 4 or 5 weeks, compensated by weekdays off. Standard therapists do not work night shifts, and on-call duties are extremely rare.
